As there is always a certain amount of paperwork required to start treatment, we have made it possible for you to download the necessary forms below. Just click on the link for New Patient Information Forms - then print it off, fill it out and return it to us by mail (P.O. Box 1927 Dalton, GA 30722), fax (706-226-1483) or in person. We will also need a copy of your insurance card if you desire us to bill insurance. Our Privacy Policy is included below as a separate link. If you want us to communicate with anyone regarding your confidential care, a link is also provided for our Release of Information Form.
