Stephanie is the Office Manager of the clinic, handling all matters pertaining to scheduling and billing (as well as a hundred other tasks). Stephanie earned her B.S. in Health Care Management from the University of Alabama (Roll Tide!). She has been with The Stevens Clinic since its inception. We believe you will find her to be both very personable and extremely competent.

As there is always a certain amount of paperwork required to start treatment, we have made it possible for you to download the necessary forms below. Just click on the link for New Patient Information Forms - then print it off, fill it out and return it to us by mail (P.O. Box 1927 Dalton, GA 30722), fax (706-226-1483) or in person. We will also need a copy of your insurance card if you desire us to bill insurance. Our Privacy Policy is included below as a separate link.
